Stockton Folk Dance Camp Registration Form

Please print these directions before downloading the form.

1. RIGHT-click on the link below to download the form to your computer. [Control-click on Macs]
2. Open the form and fill it in.
3. If you are using the most current version of Adobe Reader, on the Menu bar at the top of the screen,
   click on "Edit" and "Take a Snapshot." Caution: If you cannot find "Take a Snapshot" under Edit,
   look around in the program. Earlier versions of Adobe Reader, as well as Adobe Acrobat, have
   the Snapshot tool hidden in other menus such as Tools - Select & Zoom - Snapshot Tool.
4. Once the Snapshot tool is engaged, click anywhere on the form. You should see a flash as the software
   takes a picture of your form.
5. Open your email program and create an email to Registrar Jan Wright at jmwright32@frontiernet.net
6. Paste the form into the body of the email and send the email. If you cannot get the "Snapshot" tool to work,
   SAVE the completed form and email it as an attachment instead.

RIGHT-click on this link and then select "Save Link to" to save the Registration form to your computer.
